The Ohio FAIR Plan Underwriting Association Claims Department
will investigate claims on behalf of the Ohio Mine Subsidence
Insurance Underwriting Association (OMSIUA). Claims can be
reported using a standard Acord loss form or by electronically
submitting the Claim Notice on this web site. Coverage should be
verified by the insurance company prior to reporting the claim
and a copy of the applicable declaration page covering the date
of loss must be forwarded. All correspondence is to be directed to:

As stated above, this Association will investigate all reported
and potentially covered Mine Subsidence losses. Qualified engineers
may be requested to provide assistance. The investigation could
require subsurface investigation or the use of a survey net if the
engineer so determines. See the "Definitions" section of this site
for further information on those two investigative tools. Costs of
the investigation will be paid by the Mine Subsidence Insurance Fund.
Claim payments are made directly by the Ohio Mine Subsidence Insurance
Underwriting Association from the Mine Subsidence Insurance Fund.
Mine subsidence loss does not include loss as a result of earthquake,
landslide or collapse of storm or drain sewers or rapid transit tunnels.
Volcanic eruption and other earth movement are also excluded.
Losses will be adjusted per occurrence. All damage caused by a single
subsidence event or several events which are continuous is considered
as one occurrence.
There can be no abandonment of property to the Ohio Mine Subsidence
Insurance Underwriting Association.
